Further, I, for myself and the applicant, hereby release MPJGT, and its committe members, officers, and employees from any and all liability for any event or consequence whatsoever in any way arising out of or relating to the applicant's entry or participation on this tour with the sole and singular expectation of liability arising out of bad faith or willful misconduct. In case of any emergency occuring during any of these events, I authorize a qualified medical doctor to take necessary measures in the treatment of this applicant. At all times we have to adhear to proper golf attire, including a golf shirt with a collar. I understand that any photos taken during the tour are property of the Michigan Players Junior Golf Tour and may be used in any manner that may benefit the tour. **Caddies and automotive transportation (carts) will not be allowed. This tour is to be paid full when you mail in this application. Also keep in mind that when planning for this tour you are responsible to get players to the tournaments and do not sign up for the ones you cannot make. The reasoning is that this tour does not reimburse at the end of the season. You can carry a credit over from tournament to tournament or from year to year.
